A new break through in treating Male infertility Setting a new precedent for the rest of the country, Lilavati Hospital IVF Centre Mumbai installed a new microscope machine that allows our embryologists to pick the best quality sperm while carrying out the specialized test-tube baby procedure of ICSI.The IMSI technology was introduced in 2009 . Since then the unit has performed more than 300 plus IMSI cycles till date .We are some of the less than 50 IVF units all over the world , who have massive experience in this technology . We were the first in India . We introduced IMSI initially at the lilavati hospital unit in Mumbai and almost immediately followed it by introducing IMSI in our IVF unit at the Fortis La Femme Hospital in New Delhi. Recently in April 2011, our scientific Director Dr Hrishikesh pai was invited to the world conference on ART ,Valencia,conducted by the IVI IVF unit . He presented the units pioneering experience in IMSI . He presented a research paper , in which 90 patients undergoing IMSI were compared with 90 patients undergoing the traditional ttechnique of ICSI . All patients had either a severe male factor such as poor sperm count , poor sperm motility , abnormal looking sperms or patients with previous two recurrent IVF failures. Like the ICSI, the IMSI technique consists of introducing a sperm into the egg. The biggest difference is that in the ICSI the visualization of the sperm is increased 400 times in the ICSI classic and up to 8000 times in the IMSI. This magnification of the sperm allows for the study of structures that conventional microscopes do not permit. To achieve this, it is necessary to use a microscope of high image quality associated with complex computer software allowing for the maneuvering of the microscope from the very computer. The selection of sperm is beneficial as it is includes the observation of diverse structures within the sperm head (vacuoles, for example) along with the selected sperm quality. Depending on the size and number of vacuoles, we can determine whether or not there is damage to the DNA chain of the sperm. If the chain is affected, the use of this sperm is not allowed so as to prevent an unsuccessful pregnancy. The IMSI microinjection technique, due to its innovation, high cost and need for experienced laboratory personnel, is not used in all assisted human reproduction laboratories. It is considered a therapeutic technique of wide clinical use, as well as a possibility for research projects in collaboration with university centers making it vital for the future of assisted reproduction. RAPRUI Day Hospital (International Associated Research Institute for Human Reproduction), Via Timavo 2, Rome, Italy. monica.antinori@raprui.com The aim of this prospective randomized study was to assess the advantages of a new modified intracytoplasmic sperm injection (ICSI) technique called intracytoplasmic morphologically selected sperm injection (IMSI) over the conventional ICSI procedure in the treatment of patients with severe oligoasthenoteratozoospermia. The new procedure consisted of IMSI based on a preliminary motile sperm organellar morphology examination under x6600 high magnification. A total of 446 couples with at least two previous diagnoses of severe oligoasthenoteratozoospermia, 3 years of primary infertility, the woman aged 35 years or younger, and an undetected female factor were randomized to IVF micro-insemination treatments: ICSI (n = 219; group 1) and IMSI (n = 227; group 2). A comparison between the two different techniques was made in terms of pregnancy, miscarriage and implantation rates. The data showed that IMSI resulted in a higher clinical pregnancy rate (39.2% versus 26.5%; P = 0.004) than ICSI when applied to severe male infertility cases.
