Laparoscopy in acute bowel obstruction following previous surgery is a difficult procedure and avoided by most of the surgeons due to the difficulty in obtaining pneumoperitoneum, port placement, lack of working space, adhesions and risk of bowel injury. Here is a patient who had a previous laparotomy for trauma with a midline incision from xyphysternum to pubis; after unsuccessful conservative management he underwent a laparoscopy; a prior CT scan showed adhesions in the left side and a distal-mid small bowel obstruction. The pneumoperitoneum was obtained with the Visiport placed in the right lower quadrant; although the abdomen was grossly distended, under significant tension and distended loops of small bowel were occupying most the peritoneal cavity, with muscle relaxation there is usually enough space to perform a thorough inspection of the abdominal cavity. Port placement has to be done with special care as there is no room to push and usually a blunt trocar directed away from the bowel is employed in my practice. The collapsed loops of small bowel point quickly to the site of obstruction -- it is better to avoid manipulating the distended bowel as it is heavy, oedematous and prone to be lacerated with the instruments; once the pathology is identified, in this case the obstructive band, light packing is performed in order to expose the working space and protect the bowel from instruments like scissors or diathermy. ABSTRACT Laparascopic (or keyhole) surgery is challenging for surgeons and can be dangerous for patients. The surgeons who perform these procedures need ongoing training and hands on practice. The University Medical Centers of Groningen and Leeuwarden in the Netherlands have teamed up with game developer Grendel Games (www.surgical-games.com to produce a unique solution. Instead of using expensive, traditional simulators to teach their doctors the motor skills needed for laparoscopic surgery, they have developed a full Nintendo Wii game with customized controls that imitate a surgeon's instruments... but in a non-medical context. What makes this game unique is the choice to make a virtual game world that offers an immersive storyline, exciting puzzles and pleasing visuals, around the control scheme and motor skills of laparoscopic surgery. This deliberate change of context is unique for a medical serious game. At the 2010 Game Developers Conference (GDC), the first prototype of this game was presented. Forbes crowned it "one of the ten games that can change the world." The 2010 presentation is one of the most viewed serious games videos in the GDC vault. Your diaphragm is a muscle that separates your chest from your abdomen and helps you to breathe. Normally, the diaphragm has an opening for the esophagus to pass through where it connects with the stomach. At this point, the ring-like layer of muscle which acts as a one-way valve sometimes becomes lax. When you have reflux disease, the weakened muscle allows the contents of your stomach to back up into your esophagus. This can cause considerable discomfort, often worse at night, with symptoms like heartburn, difficulty swallowing, chest pain and belching. Reflux disease is often caused by a hiatal hernia, pregnancy, an ulcer or tumor of the esophagus. About half of the patients with severe Reflux Disease often have a hiatal hernia, which is a tear in the diaphragm.
