ICSI stands for "intracytoplasmatic sperm injection" and it's a procedure used in vitro fertilization in which the sperm is directly injected into the egg. The technique was developed by Gianpiero Palermo in 1991 and it was a real revolution in our field. Die IMSI Technik ist wie auch ICSI eine Technik zur Einführung des Spermatozoons in das Innere der Eizelle. Im Unterschied zu ICSI geht diese Technik von der Visualisierung der Spermatozoiden aus, da hierbei statt mit 400facher Vergrößerung (ICSI) mit einer bis zu 8000fachen Vergrößerung gearbeitet wird. Diese Vergrößerung der Spermatozoiden gibt Aufschluss über Strukturen, die mit einem normalen Mikroskop nicht sichtbar wären. Für die IMSI wird ein besonderes Mikroskop mit herausragender Bildqualität sowie einer komplexen Informatikersoftware benötigt, die das Bedienen des Mikroskops direkt vom Computer aus ermöglicht. Das Auswählen von stark vergrößerten Spermatozoiden ist vorteilhaft, da bestimmte Strukturen am Spermatozoonkopf beobachtet werden können (Vakuolen beispielsweise), die Aufschluss über die Qualität des ausgewählten Spermatozoons geben. In Abhängigkeit von der Größe und Anzahl der Vakuolen erhalten wir Informationen über mögliche Schäden des DNS-Strangs des Spermatozoons. Ist dieser Strang beschädigt, so kann die Verwendung dieses Spermatozoons dazu führen, dass keine Schwangerschaft zu Stande kommt oder aber in einem Abort endet. Die spermatolgische Mikroinjektion mit IMSI ist aufgrund seiner Neuheit, hohen Kosten und der Notwendigkeit von sehr erfahrenem Laborpersonal nicht in allen Laboren der assistieren Reproduktion zu finden. L'IMSI consiste à sélectionner les spermatozoïdes à l'aide d'un fort grossissement (8000 grossissements au lieu de 400 pour l'ICSI classique) et nous permet de visualiser des structures qui jusqu'ici passaient complètement inaperçues. De récentes études montrent que la sélection morphologique des spermatozoïdes a un impacte sur les taux de réussite des techniques de micro injection spermatique. La técnica de IMSI consiste, al igual que el ICSI, en introducir un espermatozoide en el interior del óvulo. La gran diferencia con el ICSI se basa en la visualización de los espermatozoides, que se aumentan 400 veces en la ICSI clásica, y hasta 8000 veces en la IMSI. Esta magnificación de los espermatozoides permite observar estructuras que en los microscopios convencionales no es posible. Para realizarla, es necesario un microscopio con una gran calidad de imagen asociado a un complejo software informático que permite el manejo del microscopio desde el propio ordenador. La selección de espermatozoides a gran aumento es beneficiosa ya que se relaciona la observación de diversas estructuras en la cabeza del espermatozoide (vacuolas, por ejemplo) con la calidad del espermatozoide seleccionado. Dependiendo del número y tamaño de las vacuolas podemos saber si hay daños en la cadena del ADN del espermatozoide. Si esta cadena está afectada el uso de este espermatozoide puede que no permita conseguir el embarazo o que éste no sea evolutivo. La técnica de microinyección espermática con IMSI, debido a su innovación, elevado coste y necesidad de personal de laboratorio con amplia experiencia no está implantada en todos los laboratorios de reproducción humana asistida.
